In the present research, the authors studied the cerebrospinal fluid IgM of patients affected by Clinical Definite/Probable Multiple Sclerosis (CD/PMS; No = 32), Clinically Possible/Suspected Multiple Sclerosis (CP/SMS; No = 15), Other Neurological Disorders (ODN; No = 55) and in 27 Healthy Subjects. Intrathecal IgM synthesis was then studied by using the IgM Index and the Reiber Formula. Normal values were lower than 0.48 for the IgM Index and 0.33 for the Reiber Formula. We separated CD/PMS and CP/SMS in two groups taking into account the disease activity: Acute and Chronic-Progressive Phases. In CD/PMS, the IgM Index resulted positive in 21.8% and the Reiber Formula showed an intrathecal IgM Synthesis in 78.1%, with a statistically significant difference. In CP/SMS, the IgM Index indicated a positivity of 26.6% and the Reiber Formula of 53.3%.
