The authors report a case of unsuspected gallbladder cancer during laparoscopic cholecystectomy for cholelithiasis. The histological specimen showed a pT2 carcinoma of the gallbladder. A second look was attempted a month later by laparotomic procedure to obtain more radical surgery. There were multiple liver metastases and peritoneal seeding. The review of the literature also shows a number of similar cases with peritoneal seeding and abdominal wall metastasis through the trocar sides. The authors suggest some diagnostic and technical procedure to prevent abdominal dissemination during laparoscopic surgery.
