The anti-ischemic effect of midterm treatment with captopril (four weeks) was investigated in 12 hypertensive patients with coronary artery disease. All patients underwent exercise stress tests at the end of one week of treatment with hydrochlorothiazide and at the end of four weeks of treatment with captopril associated with hydrochlorothiazide. Captopril reduced the S-T depression measured at the same maximal rate/pressure product and the occurrence of effort-induced chest pain, but did not change systolic pressure, heart rate, maximal rate/pressure product, total workload sustained, or total working time. Captopril may be useful in the treatment of hypertensive patients with coronary artery disease.
