This study explored the relationship between blood eosinophil counts, airway eosinophilic activation, and exacerbation phenotypes in COPD. Higher stable-state blood eosinophils were associated with increased sputum EPX, supporting EPX as a marker of eosinophilic airway inflammation. In addition, sputum IL-5 was higher in patients with eosinophilic exacerbations, including those with low stable-state blood eosinophils, indicating discordance between blood and airway markers. These findings suggest that eosinophilic exacerbators represent a distinct Th2-driven phenotype that may benefit from targeted biologic therapies.
