Intrahepatic subcapsular haematoma after laparoscopic or open cholecystectomy is a major life-threatening complication. To our knowledge, only 12 cases have been reported in literature. We herein describe three intrahepatic subcapsular hematomas, linked to one open and two laparoscopic cholecystectomies, all of them performed in patients undergoing anticoagulation or antiaggregation therapy. We discuss the necessity of a risk-benefit analysis of the actual medical "good practice" in pre-postoperative anticoagulants administration.
