Chylous ascites is the accumulation of milky fluid (chyle) in the abdominal cavity. It is an uncommon complication of retroperitoneal surgery due to obstruction or disruption of the mesenteric lymphatics or the Cisterna Chyli. As far as we know only 34 such cases, following retroperitoneal lymph node dissection (RPLND) for testicular cancer, are reported. We report a case of iatrogenic chylous ascites secondary to RPLND for post-chemotherapy residual mass that was successfully treated with only low-fat medium-chain triglyceride diet and diuretics. We describe the entero-hepatic metabolism of lipids which explains the etiopathogenesis of iatrogenic chylous ascites and relative therapeutic options.
