Secondary aortoenteric fistula is a rare complication that represents the most challenging event after reconstructive aortic surgery. Quite a few of patients present gastrointestinal hemorrhage that can leeds to life threatnig. Despite an early diagnoses and a prompt repair, the prognosis is often bleak. A 20-year experience with 7 graft enteric fistulas is reviewed. The mean interval from the original operation to the onset of symptoms was 7.7 years. Perioperative mortality was 57% and cumulative 5-year survival was 14%. Diagnosis of aortoenteric fistula is difficult and mortality is high. The right choice of the operative management still remains controversial. A high index of suspicion is the most important element of diagnoses. Prevention and early detection are mandatory.
