This study assessed the prevalence of Aspergillus sensitisation in a well-characterised COPD cohort and its association with exacerbation frequency. Aspergillus sensitisation was identified in around 14% of patients and tended to be associated with a higher proportion of frequent exacerbators, despite similar lung function and eosinophil counts compared with non-sensitised patients. Although limited by small numbers, these findings suggest that Aspergillus sensitisation may contribute to exacerbation risk in COPD and support consideration of fungal screening in selected patients.
