This study examined whether mucus plugging and chronic bronchitis represent distinct mucus-related phenotypes in young adult smokers. In the BEACON cohort, mucus plugs were uncommon but were associated with greater FEV1 bronchodilator reversibility and a higher frequency of bronchodilator responsiveness, whereas chronic bronchitis—regardless of definition—showed no such association. Blood eosinophil counts did not differ between groups. These findings suggest that mucus plugging and chronic bronchitis reflect different underlying pathological processes in early smoking-related airway disease.
